This is an external USB web camera from Logitech and it seems to work fine out of the box after a quick test.
Please note that the real name of this device as obtained from the lsusb command is Logitech, Inc. Webcam Pro 9000. I could not input that to the model name field because a comma (,) is currently not allowed to be used in that field.
